Abstract

Malnutrition improvement in Indonesia actually has been done since 1974 but percentage of malnutrition especially in Mandaling Natal city is still in high category. The health centre in Mandaling Natal which has the highest of malnutrition in 2012 was Gunung Baringin Health centre as 32 child. Therefore the improvement programme of malnutrition is needed to analysis by using health system approach. This research is descriptive survey with qualitative approach. It’s purpose is to analysis the implementation of this programme. This research was held in the working area of Gunung Baringin Health centre on Februari-May 2016. The informans were the head of Gunung Baringin Health centre, nutritionist and 25 cadre leaders of Posyandu. The result of the research showed that they were still found lack of input, process or output. Base on the input, it was because of lack of nutritionist, although they were many cadres but they were inactive  and  unskill,  and  the  lack  of  supporting  facilities  and  infrastructure.  Base  on  the process, starting fom the covering of the child growth, providing of solid food for babies, the nursing of undernutrition child was not reaching the target. So that the covering of exclusive breast feeding that was still low. These problems were caused by the law of community knowledge and awareness about the child care pattern and the lack of cadres participation in Posyandu in giving information and counselling to the community. It is suggested to Gunung Baringin Health centre staff to supervise the implementation of activities in Posyandu. It is important for training and refreshing the health officer especially the nutritionist and cadres so that they can give information and counselling to the community regularly. It is also suggested to centre and regional government, to: 1) add the nutritionist in Health centre so that the best care nutrition service can be presented to the community; 2) increase the incentive to the cadres and; 3) support facilities and infrastructure so that the cadres participation can be increased.
