Abstract

The Patient Safety Program is expected to be able to reduce the unexpected incidence that should be avoided. The policy of the Indonesian Minister of Health No. 1691/Menkes/Per/VIII/2011 stated that the hospital and health worker working for the hospital are required to implement the Patient Safety Program. Patient safety has become a major standard indicator of hospital accreditation evaluation since 2020, but in its implementation, many constraints are still found. The purpose of this study was to find out the description of the implementation, monitoring, and factors influencing the Patient Safety Program which has not been well implemented in Panyabungan General Hospital. The 11 (eleven) informants of this qualitative study were selected through purposive sampling method. The data for this study were obtained through observation and in-depth interview. The data obtained were narratively and qualitatively processed. The data collection process and data analysis were simultaneously done. The focus of this study was more on the documents of the implementation of patient safety considering that the policy is still relatively new at Panyabungan General Hospital. The result of this study showed that: the documents still less than 80%; the insufficient number of human resources; the still inadequately and totally well implemented of socialization of guidelines, instructions, and manual related to the implementation of patient safety in all units; and the inadequate provision of facilities and infrastructure. This program was supported by the director of Panyabungan General Hospital, but the support was not as expected. One of the important factors as the determinant of the implementation of patient safety was the less maximal role of the Hospital Patient Safety Team (TKPRS). The director is suggested to improve his role in determining the various policies and inadequate of provision of facilities and infrastructures. TKPRS is not multitasking and does not play an active role in doing socialization, coordination, monitoring and evaluation of the implementation of patient safety. It is expected that all units involved mutually coordinate and implement the Standard Operational Procedures related to the implementation of patient safety in order to face 2020 version of accreditation
